UPDES General Permit for Discharges from Small Municipal Separate Storm Sewer Systems (Phase II)

The phase II general permit for storm water discharges from small municipal separate storm sewer systems was issued on December 9, 2002. This permit applies to municipal storm water systems such as cities, counties, state universities, state hospitals, etc. Click the following links to view a copy of the permit and notice of intent form.

Instructions:

  1. Fill out the form completely (except for the signature)
  2. Save” the completed form as a .pdf and “send” (e-mail) the saved file to rthiele@utah.gov.
  3. Print the completed form, sign it, and send:

The Division of Water Quality has developed a Construction Storm Water Inspection Form in coordination with the SWAC organization. This inspection form (checklist) will be used by DWQ and municipalities for inspection of construction projects throughout the State. That inspection form is linked below.
