Utah Wastewater Operator Certification Program:
Applications and Forms
Welcome! You can access the Wastewater Operator Certification Application forms on this page. However, you will need Adobe Reader in order to view the forms. For a simple "renewal" please use the "Renewal" form. All other requests require the longer form. *Note: A Citizenship / Alien ID Certification form is required to be in your file as of July 1, 2009. If you have not already submitted one to DWQ, please include it with your application.
For Exams, Change of Status, and Reciprocity
Once you have Adobe Reader installed, simply click on the link below to access the application. Don't forget the due dates for exam submittals. Instructions and related fees are listed on Page 4 of the application. Please include the application with the fees. Good luck on the exam!

Note: Recent changes to R317-10 now allow for reinstatment of an expired certificate up to One Year after its expiration if the required CEUs were earned prior to its expiration.

Citizenship/Alien ID Certification
Utah Department of Environmental Quality Certification Pursuant to UCA 63G-11-104 (required for all "individuals" receiving a certification from the State as of July 1, 2009)
