OFFERED BY THE UTAH DIVISION OF WATER QUALITY
Exams may be taken on only one of the two dates - either March 6, 2009 at the Rural Water Association of Utah (RWAU) Annual Conference in St. George, OR on April 16, 2009 at one of the other three locations (Ogden, Price or Utah Valley University in Orem). Note: If there is enough demand, an April exam may be scheduled for the southern part of the state. Please contact us BEFORE submitting your application, if you wish be considered for the alternate location in April.
March 6, 2009 - 9:00 a.m. to noon, Dixie Center, St. George, Utah.
Applications must be received at the Division of Water Quality office by 6:00 p.m. on Thursday, February 5, 2009. (State offices are closed on Fridays.) This is a separate application & fee from the RWAU conference registration.
April 16, 2009 - 1:00 - 4:00 p.m., at one of the three other locations in the state: Ogden, Price, or Utah Valley University in Orem. Applications must be received at the Division of Water Quality office by 5:00 p.m. on March 19, 2009. (State offices are closed on Fridays.) This is a separate application & fee from any training seminar registration.

Note: No exam application forms or confirmation letters are being mailed unless specifically requested. The exam rosters for each location will be posted on our website under "Am I registered?" or "Exam" - "Rosters" as soon as the applications are processed. Check the rosters listed on this web site to be certain that you are scheduled for the specific type of exam and location you requested. Time allotted for the exam has been changed to three (3) hours, in compliance with ABC's testing recommendations.
COST:
$35.00 per exam - make checks payable to the "Division of Water Quality." (Full refund up to the deadline - after the deadline, no refunds can be given)

Submit a fully completed exam application and the $35.00 per exam fee (payable by check or money order, but no credit cards accepted. Do NOT MAIL cash)
